Please sign and return before the student starts school.

Disaster Preparedness Plan

In the event of a disaster should you be unable to come and pick up your child, we have provided a form on which you may designate several others who will be responsible for taking your child. Please note that no child will be released to anyone whose name is not on file with us.

Home Language Survey

The California Education Code requires schools to determine the language(s) spoken at home by each student. Your cooperation in helping us meet this requirement is requested.

Mass Media Authorization

In the event we may use mass medias such as the newspaper, television or motion pictures with your child's picture(s) or images for educational, social or other means, we would appreciate your approval to do so.

School Uniforms

All Sequoia Christian Academy students are required to wear our preferred vendor's uniforms whenever they are in class. 

Supply List

Each student is required to provide supplies for each semester of the new school year.
